• I/(Name of individual client or legal entity) (the Entity) authorize Netbank (A Rural Bank), Inc. (Netbank) to share my Personal Information (defined below) with E-Playment (the Netbank Partner). The purpose of this data sharing is to allow Netbank to open a bank account and provide me banking services through the mobile application managed by the Netbank Partner.
• I / the Entity authorize Netbank and the Netbank Partner to process, update, properly store, and maintain the accuracy of my Personal Information. The process of data sharing and updating will be consistent with Republic Act Number 10173 (the Data Privacy Act of 2012) and Netbank's Terms and Conditions (here) .
• I/the Entity waive(s) my/its rights to keeping the Personal Information confidential, provided under Republic Act 1405 (The Law on Secrecy of Bank Deposits) and Republic Act 10173 (Data Privacy Act), in favor of the Netbank Partner only. I/the Entity also authorise(s) Netbank to disclose this information to the Netbank Partner only.
• Personal Information is defined as:
  • The following information provided to Netbank / the Netbank Partner when opening my account
    • Name (Title, First Name, Middle Name, Last Name)
    • Birthdate, Birthplace, Gender, Civil Status
    • Contact Number, Email Address
    • Address
    • Income, Work Description
    • TIN and SSS number
    • Date / time of account creation
    • Customer ID generated by Netbank
    • Customer Identification File (CIF) Number generated by Netbank
  • The following details on transactions made from my account:
    • Transaction ID, Date and Time
    • Type of Transaction (Debit/Credit)
    • Status of the Transaction
    • Settlement Rail used for the transaction
    • Source Account Number and bank details
    • Destination Account Number and bank details
    • Remarks and Reference IDs from the Partner
  • The following details of my account:
    • Account Type, Number,
    • Status
    • Account holder name and Customer ID
    • Available and Current Balance. Average Daily Balance
    • Interest rate, Accrued Interest Amount
    • Maximum Balance Limit
    • Minimum Balance to earn interest
    • Required Maintaining Balance
  • Confirmation of if I hold other accounts opened in Netbank.

  The above authorizations and waivers shall apply from the start of the account opening process to the closing of the Partner Account. Further details of the Partner Account are provided in the Product Disclosure Agreements, here . Netbank’s Privacy Policy is provided here .

• I confirm that I am the account holder/the Entity's authorized representative, that I have read and understood this Consent and Waiver and am allowed to execute and deliver this Consent and Waiver.

Netbank confirms that, in its data sharing agreement with the Netbank Partner, it has received confirmation from the Netbank Partner that it is prohibited from sharing the Personal Information with any other party.
